HOW COVERAGE APPLIES

How Private Insurance Can Apply to Residential, PHP and IOP Care

A plan is applied to a level of care, and the level of care comes from clinical assessment, not from the plan. The clinical team recommends where treatment should start, and admissions looks at what the plan offers for that recommendation. Each level of care is quoted per admission, which is why no price list appears here.

Each man leaves residential care with a plan for continued treatment and support, and the steps are mapped from the start. Men move from residential care to IOP, or from residential care to outpatient counseling, as their progress allows.

Detox happens before admission and away from Guiding Road: when medically supervised detox is needed first, the team coordinates it with medical partners.

BEYOND THE LEVEL OF CARE

Coverage by Type of Care

A benefits check looks at the services inside a level of care as well as the level itself. Admissions reviews the plan against the care the clinical team recommends, which at Guiding Road includes each of the services below.

Each man has an individual therapy session every week, and a small process group meets every weekday. Therapy draws on cognitive behavioral therapy, DBT-informed skills work, motivational interviewing and other structured approaches, several of them twice a week.

Guiding Road treats substance use alongside co-occurring mental health conditions, including anxiety, depression, bipolar disorder and trauma-related symptoms. Health plans usually group mental health and substance use care under behavioral health benefits, so both are part of the same review.

A psychiatric nurse practitioner provides medication management and sees each man weekly. A physician is available, and labs and vitals are checked every week.

Families are encouraged to take part in at least two family sessions a month when clinically appropriate, and couples therapy is available when it is clinically indicated. How a plan treats family and couples therapy is worth raising on the benefits call, alongside the man’s own treatment.

The residential week includes sessions with a physical trainer four times a week, plus weekly yoga and breath work. These sit alongside the clinical program rather than replacing any part of it.

PRIVATE INSURANCE CARRIERS

Insurance Carriers and Coverage Pages

Guiding Road accepts most private insurance plans, including plans from the carriers shown here. Ten of them have their own coverage page. For the others, and for any private plan not shown, admissions checks the plan on the benefits call.

THE BENEFITS CHECK, STEP BY STEP

What the Benefits Check Covers

Cost is a fair question to settle before choosing treatment. The benefits check settles it while the family is still deciding, so the program can be weighed on its merits with the cost questions already answered.

Step 1
Call Admissions
The check starts with a call to admissions, and the line is answered 24 hours a day.
Step 1
Step 2
Share the Plan Details
The caller shares the plan details listed below, and the team looks up the plan's benefits for addiction treatment.
Step 2
Step 3
Review What the Plan Offers
Admissions explains what the plan offers for the recommended level of care. Questions about what the family may pay are answered on this call, for this plan.
Step 3
Step 4
Decide on Next Steps
With the benefits explained, the family and the man decide what comes next. The conversation also covers fit: the program is abstinence-based and 12-step work is part of it, so both are discussed before admission.
Step 4

FOR THE FIRST CALL

What to Have Ready for the Call

A few details let the team look up the plan while the caller is still on the phone. A family member who does not have all of them can still call: admissions explains what information is needed next.

INSURANCE TERMS, EXPLAINED

Insurance Terms Families Hear on the Benefits Call

These are the words that come up once a plan is being reviewed.

A set amount paid for a covered service, such as a visit or a day of treatment.

Out-of-Pocket Maximum

The yearly cap on what the member pays toward covered care under the plan’s terms. Deductibles, copays and coinsurance all count toward it.

Checkmark icon

Prior Authorization

Approval from the plan before certain care begins. Residential treatment is a level of care where plans may ask for it.

Heartbeat line icon

Medical Necessity

The standard a plan uses to decide whether care at a given level is needed, based on the clinical assessment and the plan’s own criteria.

At Guiding Road, admissions walks through each of these for the family’s own plan on the benefits call, in plain terms, before any decision about admission is made.

LIMITS AND DENIALS

Why a Plan Might Limit or Deny Care

Plans decide what they pay for using their own rules. Knowing the usual reasons ahead of time makes the benefits call easier to follow, and it means a limit is heard about before admission rather than after.

When a plan turns down a request, the member can ask the plan to reconsider through its appeal process. At Guiding Road, admissions goes over the plan’s rules on the benefits call and explains any limit in plain terms, so the family knows what applies before deciding.

WHAT TREATMENT LOOKS LIKE HERE

A Small Men's Treatment Program With Personal Attention

Insurance settles how treatment is paid for. The reason to choose Guiding Road is what that treatment looks like: a small men’s program where the team gets to know each man and the men get to know each other.

Mark North founded Guiding Road with his own experience of recovery and a background in hospitality. Both shape a program where men get to know one another, take part in treatment together and prepare for life beyond it.

Personal Attention

The Phoenix house has 10 residential beds, a direct-care staff ratio of 1:3 and a clinician ratio of 1:6. The psychiatric provider sees every client on Mondays and the physician on Wednesdays, which gives the team time to notice how each man is doing.

Men Working on Recovery Together

Shared meals, small process groups, service work and honest conversations fill the week, and men have the chance to build friendships that can last beyond treatment.

Support for the Family

Family support follows a curriculum on communication, boundaries, relapse warning signs and rebuilding trust, so the people at home are part of the work while treatment is under way.
